Home health care is for individuals who may benefit from skilled services while recovering from an illness, injury, or surgery. It enables individuals to remain in the comfort of their own home or assisted living facility close to their loved ones.

Home health care may be right for you or your loved one if the following criteria are met:

• Need for either intermittent or part time skilled nursing care or therapy.

• Are homebound and unable to get to the physician. 

​

• An individualized plan of care must be ordered and approved by the attending primary care physician.
